Grey County Long-Term Care homes embark on Clinical Pathways program journey
Grey County Long-Term Care homes are embracing a three-year initiative to implement the Registered Nurses Association of Ontario (RNAO)’s Clinical Pathways program, aimed at enhancing resident care.
This innovative program offers a structured approach to care delivery in long-term care homes, aligning with RNAO's best practice guidelines. By integrating Clinical Pathway tools into electronic medical records, the homes aim to streamline documentation and ensure consistent, high-quality care for residents.
Staff members have undergone comprehensive training and preparation, including gap analyses, policy and procedure development, and communication planning. Their efforts culminate in the April 30 rollout of foundational tools including admission assessment, delirium assessment and resident and family-centered care.
“Clinical Pathways is an exciting opportunity to improve resident health and well-being in Grey County's long-term care homes,” said Denna Leach, Long-Term Care Clinical Specialist. “Since November, personal support workers, dietary and recreation team members, nursing teams and leadership have been focused on preparing for this launch.”
Jennifer Cornell, Director of Long-Term Care, commended the project team's dedication to the project. “We’re proud of our project team’s efforts to Colour it with Clinical Pathways and build on our commitment to person-centred care.”
Future phases of the program will introduce additional Clinical Pathways focusing on falls, pain management, palliative care, and clinical guidelines.
